Applying for or Renewing a Medical Cannabis Card

 Medical cannabis cards can only be obtained from the Department of Health and Human Services.

Utah Patients Coalition has provided a guide for potential patients on how to obtain a medical cannabis card via the DHHS.

Patient Card

Patients 21+ years old and have a diagnosed qualified condition may apply for a medical cannabis card in Utah. Most patients in Utah will use this method of obtaining and renewing a medical cannabis card.

Find a QMP

Find a list of QMPs (qualified medical provider) near you here.

Meet with QMP

The law requires initial QMP visits to be in person.

Create a Utah ID Account

If you don't already have a Utah ID account, you can create one here. Or view the tutorial here.

Apply Online

Apply for a medical cannabis card online using the EVS (electronic verification system).

Wait for Certification

After creating a profile, the medical provider will be notified and enter a certification and recommendation.

Pay $15 Fee

Open your EVS profile and pay the $15 fee to the UDOH.

Review

DHHS reviews the application in 15 days or less.

Card Emailed

If approved, DHHS will issue the medical cannabis card and email a copy to the patient.

Print Card

To purchase medical cannabis, bring a copy of the medical cannabis card and valid photo ID to a medical cannabis pharmacy, and consult with a pharmacist if required. Locate a cannabis pharmacy here. 

Minor/Guardian Card

Medical cannabis guardian cards are issued simultaneously with provisional patient cards (cards for those patients younger than 18).  Using the process below, both the guardian and the minor will obtain a card.

Open your EVS profile and pay the fee(s) applicable.
Guardian and Provisional Patient Cards initial: $68.25 renewal: $24
Secondary Guardian and Provisional Patient Cards initial and renewal: $15

Medical records to Compassionate Use Board (CUB)

Provide medical records related to the recommending condition to the QMP to be sent to the CUB. 

Background Check

 After payment,  guardians begin the background check process via an email from the DOH.

Review

DHHS reviews the application in 15 days or less.

Card Emailed

Once approved, receive both the medical cannabis guardian card and the provisional patient card via email.

Print Card

To purchase medical cannabis, bring a copy of the medical cannabis card and valid photo ID to a medical cannabis pharmacy, and consult with a pharmacist if required.

Caregiver Card

The patient of the caregiver must already have a medical cannabis card. This process will only qualify a caregiver to order, pickup, transport, and receive cannabis for the patient. 

Patient Must Designate Caregiver

Before a caregiver begins the process to apply for a medial cannabis caregiver card, a patient may add the caregiver’s information to the electronic verification system (EVS).

Apply for a medical cannabis card online using the EVS (electronic verification system).

Pay Fee to UDOH

Open your EVS profile and pay the fee(s) applicable.
Caregiver Card initial: $68.25 first renewal: $0 Subsequent renewals: $14
Secondary Patient Cards initial: $15

Background Check

After payment,  caregiver begins the background check process via an email from the DOH.

Review

DHHS reviews the application in 15 days or less.

Card Emailed

The caregiver will receive their card via email. The caregiver can then save the card on their smartphone or print it out.

Purchase

To purchase medical cannabis, bring a copy of the medical cannabis card and valid photo ID to a medical cannabis pharmacy, and consult with a pharmacist if required.

Non-Utah Resident Card

If you have a medical cannabis card from another state and you're visiting Utah, you can apply for a temporary card to access Utah Medical Cannabis pharmacies. The temporary card is valid for 21 days at a time and you can get two cards per year. However, you must have a qualifying condition as defined by Utah law to be eligible for the temporary card.
