Medical Cannabis Cards
All patients must hold a valid medical cannabis card to legally purchase and possess a medical cannabis product in Utah.

Applying for or Renewing a Medical Cannabis Card
Medical cannabis cards can only be obtained from the Department of Health and Human Services.
Utah Patients Coalition has provided a guide for potential patients on how to obtain a medical cannabis card via the DHHS.
Medical Cannabis Patient Card
- Issued to patients 18 years of age and older
- Valid for six months upon initial
issuance, then six months upon renewal. - Card for patients age 18-20 must have card approved by Compassionate Use Board
How to Get a Medical Cannabis Patient Card in Utah
QMP
Find a medical provider who can recommend medical cannabis.
Consultation
Meet with the provider in person or video call to qualify for a medical cannabis card.
Utah ID
Create a Utah ID account online if you don’t already have one.
EVS
Apply for a medical cannabis card online using the EVS Patient Application Guide.
Verification
After creating a profile, the medical provider will enter a certification and recommendation.
EVS Fee
Pay the $15 medical cannabis card application fee online.
Approval
Most cases, a temporary card is issued for immediate use. DHHS will issue the medical cannabis card soon after and email a copy to the patient.
Purchase
To purchase medical cannabis, bring the medical cannabis card and valid photo ID to a medical cannabis pharmacy, and consult with a pharmacist if required.
Medical Cannabis Guardian / Minor Card
- • Issued to parent or guardian of patients
under 18 years of age
• Valid for 60 days upon initial issuance,
then six months upon renewal (with
additional fee)
• Criminal background check

Once approved, receive both the medical cannabis guardian card and the provisional patient card via email.

The patient with an active medical cannabis card designates the caregiver online.
Utah ID
The caregiver creates a Utah ID account online in their name if they don’t already have one.

Review the caregiver EVS user guide and apply online for a medical cannabis card
EVS Fee
Pay the $15 medical cannabis card application fee online.
Background Check
Complete background check forms after receiving an email from the Department of Health.
Application Review
DHHS reviews the medical cannabis card application, and it takes up to 15 days after the background check is cleared.
Approval
Once approved, receive the medical cannabis caregiver card via email.
Purchase
To purchase medical cannabis, bring the medical cannabis card and valid photo ID to a medical cannabis pharmacy, and consult with a pharmacist if required.
Medical Cannabis Non-Utah Resident Card
Patients who are not Utah residents or
who have been residents for less than
45 days
• Must have a currently valid medical
cannabis card or the equivalent of a
medical cannabis card under the laws of
another state, district, territory,
commonwealth, or insular possession
of the US
• Must have been diagnosed with a
qualifying condition
• Issued for no more than two visitation
periods per calendar year of up to 21
calendar days per visitation period
How to Get a Medical Cannabis Guardian Card in Utah
Utah ID
The caregiver creates a Utah ID account online in their name if they don’t already have one.
EVS
Review the Non-Utah resident EVS user guide and apply online for a medical cannabis card.
EVS Fee
Pay the $15 medical cannabis card application fee online.
Approval
Receive the medical cannabis card via email the day before your travel to Utah. Make sure to save it on your device or print it out. You can call 801.538.6504 to verify receipt.
Purchase
With the medical cannabis card, purchase medical cannabis from a medical cannabis pharmacy in Utah. Bring your card and a valid photo ID, such as a driver’s license, to the pharmacy. Before making the first purchase, consult with a medical cannabis pharmacist. Some pharmacies may require an appointment in advance for the consultation.
